For general information, or to schedule appointments for Woman Infants and Children (WIC), Well Woman HealthCheck, or Family Planning at one of our Clinic Locations, please call the Central Appointment Desk toll free at 1-866-960-0633, Monday through Friday from 8:00am to 5:30pm.

For birth certificates or death certificates, please call Vital Records via our toll free directory at 1-800-231-8499.

To report a public health emergency or 24-hour reportable disease:
During normal business hours ask for the Public Health Duty Officer at: 520-866-7325 (main number) or 520-866-2929 (fax). If after business hours, call 520-866-6239 and ask for the Public Health Duty Officer.
